CMS GUIDE Model
Caregiving doesn’t have to be a one-person job.
Missoula Aging Services is proud to partner with Montana Stillwater Health to bring the CMS GUIDE Model to our community.
The GUIDE Model is a program designed to improve the quality of life for people living with dementia while providing education, support, and practical resources for the family members and friends who care for them. Through this partnership with the Centers for Medicare and Medicaid Services, participants receive coordinated, person-centered care that helps them remain safely at home for as long as possible. Because the GUIDE Model is a CMS Innovation Project, it will run through June 2032.
What services are available?
Patients enrolled in the GUIDE Model may receive:
- Comprehensive dementia care planning
- Ongoing support from a dedicated Care Navigator
- Care coordination between healthcare providers and community services
- Education and coaching
- Connection to local resources and supportive services
- Access to respite care for eligible caregivers
Who is eligible?
To participate, individuals must:
- Be enrolled in Original Medicare (Part B)-not a Medicare Advantage plan
- Have a diagnosis of dementia, or receive one through the clinical assessment process
- Live in the community or in an assisted living facility
Who is not eligible?
At this time, the GUIDE Model is not available for individuals who:
- Are receiving hospice services
- Reside in a memory care unit
- Reside in a skilled nursing facility (nursing home)
How to Enroll
- Let us know you’re interested: Call Missoula Aging Services at (406) 728-7682 for an initial GUIDE eligibility screening.
- Assessing your needs: Complete a comprehensive clinical assessment with Montana Stillwater Health.
- Care planning: Meet with a Missoula Aging Services Care Navigator for an in-home care planning visit.
- Support in place: Receive ongoing care navigation, education, care coordination, and caregiver support through the GUIDE Model.
